DUTCH hormone testing is a laboratory method that uses dried urine collected at specific times. A comprehensive panel may include estrogen and progesterone metabolites, selected androgens such as DHEA-related markers and testosterone metabolites, and cortisol or cortisone measurements across the day. Displaying these markers together may help a clinician explore selected hormone relationships as well as metabolism and clearance pathways.
Timed urine sampling can show selected markers over several collection points, and dried cards may be more convenient than a traditional 24-hour urine container. Urine also contains metabolites that are not measured in the same way through every blood or saliva panel.
The test still needs a defined purpose. More measurements do not automatically create a more accurate diagnosis, and a ratio outside a reference range does not by itself prove that symptoms come from a hormone “imbalance.” A detailed report is a map, not the destination.
Why One Hormone Number May Not Tell the Whole Story
Hormones are produced, transported, converted, broken down, and excreted through connected pathways. They also influence one another through feedback systems, and their effects depend on receptors, binding proteins, tissue sensitivity, cycle or menopause stage, medications, and timing.
When I use the word “balance,” I do not mean that every hormone should be equal or fit one universal ideal ratio. I mean considering selected hormones in relationship: estrogen with progesterone, androgens within the broader steroid-hormone picture, cortisol with cortisone, and all of these patterns within your symptoms, cycle or menopause stage, medications, and health history.
DUTCH testing may also show selected metabolism and clearance pathways—the steps through which parent hormones are converted into metabolites and prepared for excretion. These are sometimes called “detox pathways.” Here, detoxification means normal biochemical processing and clearance; it does not mean a commercial cleanse, and one urinary pathway does not by itself prove that your body is “toxic” or failing to detoxify.
This matters in perimenopause because ovarian hormone production becomes more variable before it eventually declines. The Menopause Society’s overview of perimenopause explains that erratic ovarian hormone production and less-frequent ovulation can change menstrual flow and cycle length. Cholesterol is the starting material for steroid hormone production. That biochemical fact is interesting, but it does not mean that one low cholesterol result proves low adrenal hormones or explains fatigue and brain fog. Those symptoms have many possible causes and deserve a complete evaluation.
This is a central principle of DUTCH hormone testing: the information can be broader, but it must not become a shortcut around careful medicine.
How Dried Urine Testing Works
The collection schedule depends on the exact panel and laboratory instructions. A typical multi-point protocol uses several samples from waking through bedtime. Some protocols add a sample when a person wakes during the night; others combine dried urine with saliva. Cycle-mapping protocols collect on specified cycle days rather than only one day.
Follow the kit instructions exactly. Collection timing, fluid intake, caffeine or alcohol exposure, sleep schedule, menstrual-cycle timing, medications, hormone therapy, supplements, and kidney function may affect results or their interpretation. Do not stop prescription medication or hormone therapy for a test unless the prescribing clinician specifically instructs you to do so.
Research has examined whether dried urine can reliably measure selected markers. A 2019 study reported useful agreement for selected estrogen and progesterone metabolites. A 2020 study found strong agreement between dried and liquid urine measures of cortisol-related markers and found that four spot samples could represent a general diurnal pattern. A 2021 study also evaluated repeat reliability.
These findings support analytical and sampling reliability for selected measurements. They do not prove that the complete panel improves outcomes or is necessary for every question. The validation studies involved investigators affiliated with, or supported by, the test manufacturer, so independent replication and outcome research remain important.
Here are seven ways the test may add context when it is appropriately chosen.
1. It May Show Selected Estrogen Metabolism and Clearance Pathways
“Estrogen” is often discussed as though it were one substance. The body contains several estrogens, including estrone, estradiol, and estriol, along with downstream metabolites formed as these hormones are processed.
DUTCH hormone testing may display selected estrogen metabolites and ratios related to phase-one hydroxylation and phase-two methylation. These are parts of the body’s normal estrogen metabolism and clearance. The pattern may prompt questions about liver processing, medication exposure, nutrition, alcohol, bowel regularity, and other influences on metabolism and excretion.
However, this information must be handled carefully. A metabolite is not automatically “good” or “bad,” and one pathway does not prove that a person is “toxic” or unable to detoxify estrogen. Urinary metabolites show excreted products. They do not by themselves tell us how much active hormone is reaching every tissue or whether a metabolite caused a symptom.
I use the results as context rather than a verdict. If the information does not connect to your history, risks, and a reasonable plan, it should not drive treatment by itself.
2. It May Add Information About Progesterone Metabolites
Progesterone changes with ovulation and life stage, and its relationship with estrogen is part of the clinical picture. In cycling women, timing is essential because both hormones vary across the month. A sample taken without attention to cycle day can be difficult to interpret.
DUTCH hormone testing commonly measures urinary metabolites of progesterone rather than serum progesterone itself. That may add context when a clinician is exploring whether the timing and pattern are consistent with ovulation or monitoring a therapy. It is not a stand-alone fertility test and does not determine egg quality, tubal patency, implantation, or a partner’s reproductive health.
If infertility is the concern, a complete evaluation matters. The American Society for Reproductive Medicine recommends evaluating ovulatory status, the structure and patency of the female reproductive tract, and semen when applicable. A hormone-metabolite panel may be one piece of a much larger question; it should not delay established fertility care.
3. It May Broaden the Androgen and DHEA Conversation
Women need androgens too. DHEA is produced largely by the adrenal glands and can serve as a precursor for other sex steroids in tissues. Testosterone and its metabolites can also be relevant to libido, body composition, hair or skin changes, and certain endocrine questions.
DUTCH hormone testing may include DHEA-related markers and testosterone metabolites, offering a wider view of androgen metabolism. That can be useful when a clinician is already considering a broader differential diagnosis.
It is not permission to start DHEA or testosterone automatically. DHEA can affect androgen and estrogen pathways and is not appropriate for everyone. The Endocrine Society’s adrenal hormone resource notes that many anti-aging claims remain unproven and that more research is needed on benefits and long-term risks.
Likewise, a cortisol-to-DHEA relationship may be interesting, but it is not a universally accepted stand-alone diagnosis. I interpret these markers conservatively and in context.
4. It May Show a Representation of Cortisol and Cortisone Across the Day
Cortisol is often described as the “stress hormone,” but that label is incomplete. Cortisol helps regulate metabolism, blood pressure, inflammation, energy availability, and adaptation to stress. Your body needs it.

Shift work, illness, sleep disruption, medication, psychological stress, and other factors can influence the pattern. A timed profile may add context when a clinician is exploring why symptoms appear at particular times.
DUTCH hormone testing may measure free cortisol, cortisone, and cortisol metabolites. Looking at these together may add context about daily rhythm, total metabolite output, and the conversion between cortisol and cortisone. The 2020 dried urine study found that a four-spot collection reflected the general diurnal pattern seen in saliva, although urine timing is shifted because hormone production and urinary excretion do not occur simultaneously.
A urine profile is not identical to a blood or saliva curve, and it does not diagnose Cushing syndrome or adrenal insufficiency. The Endocrine Society’s Cushing syndrome guideline recommends established tests for suspected disease, such as 24-hour urinary free cortisol, late-night salivary cortisol, or dexamethasone suppression testing. Suspected adrenal insufficiency also requires standard endocrine evaluation.
If a report is described as proof of “adrenal fatigue,” pause. That term is not an established medical diagnosis. Symptoms are real, but they need a careful search for sleep disorders, anemia, thyroid disease, medication effects, depression, metabolic concerns, infection, and other possible causes.
5. It May Connect Symptoms With Timing
One of the most practical ideas in the transcript is that timing matters. “I am tired” is important. “I wake refreshed, crash at 3:00 p.m., feel alert again after dinner, and cannot fall asleep” gives a clinician a more useful pattern to explore. DUTCH hormone testing may add time-of-day cortisol information, but the test becomes more meaningful when it is paired with your real-life timeline:
- When do you wake, eat, exercise, use caffeine, and go to bed?
- When do symptoms begin and improve?
- Do they change across your menstrual cycle?
- Are there work shifts, caregiving demands, travel, pain, or nighttime awakenings?
- Which medications, hormones, or supplements are used, and at what time?
This does not prove that cortisol caused the pattern. It lets us compare a laboratory representation with lived experience. Sometimes the most valuable outcome is a sleep assessment, medication review, nutrition support, or referral – not a supplement.

A comprehensive report can look definitive. It is not.
On its own, DUTCH hormone testing cannot:
• Diagnose perimenopause in every woman or determine the exact date menopause will occur
• Identify the single cause of fatigue, brain fog, anxiety, weight change, or insomnia
• Confirm or exclude PCOS, infertility, thyroid disease, Cushing syndrome, or adrenal insufficiency
• Prove that an estrogen metabolite is causing cancer or that a supplement will prevent it
• Determine whether hormone therapy is safe for you without a complete risk assessment
• Replace evaluation of abnormal bleeding, pelvic pain, breast symptoms, severe headache, chest pain, fainting, or other concerning changes
• Turn a population reference range into a personalized treatment target automatically
• Define one universal “perfect” hormone balance or prove that a single ratio is causing your symptoms
Women age 45 or older with typical symptoms often do not need hormone testing to identify the menopause transition. NICE guidance notes that laboratory tests generally do not help diagnose perimenopause or menopause in otherwise healthy people in this age group because levels fluctuate. Testing may still be appropriate for younger women, atypical symptoms, possible pregnancy, suspected alternative conditions, or decisions that depend on a result.
